Diabetes affects the beta cells of the pancreas, which produce insulin. In type 1 diabetes, the immune system destroys these cells, which severely reduces or stops insulin production, which in turn raises blood sugar. In type 2 diabetes, the beta cells are still present, but they do not produce enough insulin due to insulin resistance and overload. Over time, the function of the beta cells also decreases here, resulting in an insulin deficiency.

The preservation and regeneration of beta cells are crucial for the treatment of both types of diabetes. Studies suggest that CBD (cannabidiol) from the cannabis plant offers promising therapeutic potential, especially for type 2 diabetes (adult-onset diabetes), as it improves insulin sensitivity and reduces inflammation.

Study Situation: Diabetes 2 and CBD

In type 2 diabetes, the central challenge is insulin resistance. Body cells react less effectively to insulin, leading to elevated blood sugar levels. Chronic inflammation exacerbates this problem.

CBD interacts with the endocannabinoid system (ECS), which regulates many processes in the body, including inflammation, glucose metabolism, and neuronal functions.¹ Current research suggests that CBD could therefore also help with diabetes:

It was found that an overactivation of the endocannabinoid system (ECS), especially signaling via cannabinoid receptor 1 (CB1R), plays a crucial role in the development of insulin resistance and thus diabetes (especially type 2). Since CBD interacts with the ECS, it is suspected that CBD could have a positive influence on diabetes and associated concomitant symptoms.²

Furthermore, it was found that the anti-inflammatory effect of CBD can help alleviate the chronic inflammation associated with diabetes and contributing to insulin resistance. In animal experiments, researchers showed that the incidence of diabetes decreases when mice are treated with cannabidiol.³

CBD for Type 1 Diabetes

Unlike Type 2, Type 1 diabetes is an autoimmune disease where the immune system attacks the insulin-producing cells of the pancreas. Here, the immunomodulatory properties of CBD could be significant. Studies in mice showed that CBD can dampen immune responses and thus stabilize pancreatic function.⁵ 

However, studies in mouse models also showed undesirable effects of CBD application in Type 1 diabetes, especially in patients with complications like diabetic nephropathy.⁶ Therefore, it is particularly important for patients with Type 1 diabetes to consult their doctor before using CBD.

Side Effects of CBD in Diabetes

In addition to the known, mild, and short-term side effects that CBD can have in some cases (fatigue, dizziness, digestive problems), CBD can have other unwanted side effects for diabetes patients. CBD can affect enzymes in the liver and thus alter the effect of other medications, such as blood sugar-lowering agents. Diabetics should therefore always consult their doctor before taking CBD products.

Due to its anti-inflammatory and potentially blood sugar-lowering properties, CBD oil offers an exciting possibility to support diabetes treatment. Although current research results are promising, large-scale clinical studies are still lacking to determine optimal dosage and long-term effects.
